EYHA 2017-2018 REGISTRATION CHECKLIST

STEP 1

USA HOCKEY REGISTRATION 
 
Follow these steps to register online with USA Hockey:  
 
1. Go to www.usahockey.com and click on the “register, online” button.
2. Choose member type Ice Player/Coach.
3. You must be 18 years of age to process a registration…. Check the box that acknowledges that YOU are 18 or older.
4. Select the correct season – 2017-2018.
5. Follow the prompts…fill in all data correctly.
6. Process payment – Visa, Master Card, Discover or American Express (credit or debit card). A Visa gift card will also work.
7. You may print your Confirmation page at this time. It will also be emailed to you immediately after the registration is completed.
 
You will need the confirmation number to enter into your player's registration when completing the on-line registration for the below program.

STEP 2

NEW TO EYHA - You will need to first register online at EYHA. 

  • If you have not registered at EYHA via ONLINE before, please register as a New Parent.   
  • This will cause an e-mail to be sent to you with a password (for security purposes). 
  • Continue the registration, entering information about YOU (the adult) first.  
  • Once registered as a member, add adults (your spouse for example) and players to your membership.  
  • VERY IMPORTANT: You must select the REGISTER button next to your child's name to proceed with the registration process.

Returning Member to EYHA.

  • Log into your personal account with your email address and password.
  • VERY IMPORTANT: You must select the REGISTER button next to your child's name to proceed with the registration process.

To register for a program, click on the form title and fill out the required information. 

  • Be sure to "accept" any boxes requiring acceptance by clicking on the box. 
  • All fees are collected electronically via credit card or bank transfer through secure online processing.  
  • By paying with bank transfer there is tremendous cost savings to the league.

STEP 3

Once you have registered the EYHA registration, you can either pay your deposit or pay your balance. 

  • Your registration is not complete until your deposit has been paid.
  • Player balances must be paid by October 1, 2017.

STEP 4

Player Paperwork

  • NEW for 2017-2018, ALL player paperwork must be printed, filled out, signed and brought to your players first evaluation check-in.
  • Your player will not be allowed to take the ice without the necessary completed paperwork.

Player Birth Certificate

  • ONLY if you are new to EYHA and we don't have your birth certificate on file.
  • There are two options for submitting your players birth certificate:
    • Scan a copy to your players account.
    • Bring a copy of your players birth certificate with your paperwork to the first evaluation. 

ALL PAPERWORK MUST BE BROUGHT TO YOUR PLAYERS FIRST EVALUATION OR THEY WILL NOT BE ALLOWED TO TAKE THE ICE.
